Pre-OCR01000 Removal Process (Lockdown 1)

Prior to the posting of OCR01000, a sitewide review of everything posted to date was performed, with suggestions accepted by listeners and reviewed by the judges panel for both standards violations and quality issues, with a stronger emphasis on the former. Due to the quantity of removals, they are listed here not by category of the reason they were removed, but as a group.
